Adding a Form to a Channel

Once you have created your Form Templates in the Admin Area you will want to add them to an open channel. You can also add them to Collections and include them in a Play.


To add forms in a channel:

  1. Access the open Channel
  2. Go to Forms
  3. Click on the orange Add button
  4. Select the form you wish to add to the event / incident


To add forms in the channel:

  1. Open the Incident Management Mobile App
  2. Access the open channel
  3. Select Forms
  4. Click on the orange + icon
  5. Select a form from the list
  6. Click Create
