Creating a New Form

You can quickly and easily replicate paper forms that your organization utilizes allowing simple completion in an online, collaborative and completely auditable environment such as ICS forms, a meeting schedule, action plan, executive summary etc. in your Incident Management account (Operations Center platform).

🔑 ACCESS LEVEL: Account Owner

💡 TIP: Names should be unique and clear. For example, EMT Meeting Agenda, ICS 208 Safety Message, Hazardous Materials Report, etc. The name will be the name of the form that appears in the forms module within a channel and will be the header on the PDF printout of the form. 

To create a new form:

  1. Go to the Admin Area
  2. Expand Templates
  3. Select Forms
  4. Click on New form template
  5. Fill in the details and create your template using the customizing form template and selecting field type articles
  6. Click Save
